Possible Ways to Extend Coverage—For Those Not Eligible for U.S. Medicare Only
If you are not eligible for U.S. Medicare your coverage may be extended beyond the time it would otherwise end in certain situations. The Plan provisions under which this extended coverage are available are:
- Total Disability - Extension of Medical Plan Benefits for Disabled Individuals Only;
- COBRA Continuation Coverage;
- Purchase of an Individual Insurance Policy From an HMO (if available).
Total Disability - Extension of Medical Benefits for Disabled Individuals Only
If you or a covered dependent are not eligible for U.S. Medicare and are Totally Disabled and under the care of a physician when your coverage terminates, you or your dependent may continue to receive medical benefits under this Plan, for covered services related to the illness or injury which caused the total disability. To receive benefits under this provision, an individual must apply within 31 days of loss of coverage and provide necessary documentation of the total disability to the trust. Benefits under this provision will be extended for the period of disability but not to exceed eighteen (18) months.
Benefits provided under this section will end if the disabled individual ceases being Totally Disabled, if benefits paid reach the lifetime maximum under the Plan, or the Plan terminates.
When this extension applies, it will operate only to the extent that coverage for medical care is not otherwise provided for the person through the Trust.
