Grievance Procedures
If you or a dependent have any complaint regarding services or benefits under this program, you may contact by telephoning ’s Access Line or writing . ’s address and telephone number are listed below. You will be furnished with a grievance form which you may file with to initiate ’s grievance resolution procedures. ’s staff including, as necessary, its Director of Provider Relations, will work with you to resolve the grievance and will inform you of its decision. If you are still dissatisfied, you have the further right to invoke the appeals procedures set forth in this Plan booklet. Call your Area Administrative Office for further information and assistance.
Coordination of Benefits
If you or your dependents have Mental Health or Chemical Dependency coverage under another plan, benefits under this plan may be coordinated with benefits payable under the other plan.
Public Policy
An eligible participant or dependent who has suggestions regarding public policy or has concerns regarding any aspect of ’s administration that relates to public policy may address a letter to the Board of Directors at the address listed below. The Board will investigate and respond to all communications.
